Replaced clutch, thowout bearing and flywheel. The disc wore out prematurely, resulting in hot spots on both the flywheel and pressure plate. It was an aftermarket item that had failed. The root cause is still unknown. McLeod (clutch manufacturer) and Clutch Masters (slave cylinder manufacture) both said the other product caused the failure.

The coolant overflow tank cap was not put on tight by Grease Monkey and I was running dry on coolant but not overheating. But, my MPG was really poor due to MAF sensor being thrown off by low coolant. Filled up and monitored...fine now.
