2010 Toyota 4Runner 4dr SUV

270-horsepower 4.0L V6 5-speed shiftable automatic 4WD, part-time w/low range

40400 mi At a full stop with breaks applied, occasionally the car would lurch. Toyota replaced the drive shaft and it has not occurred since. I believe there was a Toyota bulletin issued sometime prior to my incident. 
22000 mi Drive shaft replaced (TSB) because it was knocking when braking or when taking off from a stop sign or light. Problem fixed.

Park Neutral safety switch faulty. Had a soft/mushy feeling when moving transmission lever, especially from Park to Drive. Would sometimes shift itself out of drive and into neutral, while driving. Known issue with this sensor. Small but expensive part to replace.
