47300 mi Was using coolant with no visible leaks, found rear cylinder bank head gasket was leaking. Replaced both head gaskets. 
15000 mi Slow coolant leakage suspected by owner. Had missing coolant problem since van was bought. Explained problem to dealer but pressure tests show no evidence of coolant leakage. I suggested the leak may be internal. Return next time.
