Headlight adjusters on the e39 cars will often fail due to brittle plastic. Repair kits are available on Amazon and ebay, but replacement of the adjuster is difficult, as it involves completely disassembling the entire headlight assembly. Pre-2001 5-Series have more easily serviceable clamshell headlights. Post 2001 models must be heated along a seam to gain access to the internals of the headlight for replacing the adjuster.
